Common causes of Shoulder pain and discomfort:

Frozen Shoulder (Adhesive Capsulitis)
A condition characterized by inflammation and thickening of the shoulder joint capsule. The shoulder joint capsule is a connective tissue structure that surrounds the shoulder joint, and in frozen shoulder, it becomes tight and stiff. This leads to pain, stiffness and gradual loss of range of motion in the shoulder.
Rotator Cuff Strain
Strains in the rotator cuff commonly contribute to shoulder pain, resulting in discomfort and restricted range of motion. Rotator cuff strains are commonly caused by lifting heavy objects (i.e. bench press or sports), poor posture, or overuse, repetitive motions.
Muscle Tightness
Muscle tightness is a sensation of tension or stiffness in the muscles around the shoulder region. It can result from poor posture, muscle imbalances, overuse, stress, or trauma. Causing discomfort, aching, or soreness in the shoulder muscles.
Shoulder Tendinitis
Inflammation in the shoulder, often caused by overuse, repetitive motions, or injury, leads to Shoulder Tendinitis, causing pain and discomfort.
Shoulder Impingement
This occurs when the rotator cuff tendons are pinched or compressed during shoulder impingement, causing pain, particularly when raising the arm. It commonly arises during exercises or sports activities.
Osteoarthritis
A degenerative joint disease affecting the shoulder joint, Osteoarthritis induces pain, stiffness, and a diminished range of motion.
When Shoulder Pain Needs Urgent Care
Most shoulder pain can be safely assessed and managed by our GP team. However, please seek medical attention promptly, or visit an Emergency Department, if you experience:
- Sudden, severe shoulder pain after a fall, accident, or direct blow
- Visible deformity of the shoulder or inability to move the arm at all
- Numbness, tingling, or weakness spreading down the arm
- Shoulder pain accompanied by chest tightness, breathlessness, or pain spreading to the jaw or left arm (these can be signs of a heart-related emergency, not a shoulder problem)
- Pain that is rapidly worsening despite rest
In the above instances, it would be highly recommended by doctors to rule out more serious shoulder injuries through appropriate imaging, such as an X-ray, ultrasound, or MRI, and for the patient to be assessed by an Emergency Physician or Orthopaedic Surgery Specialist before starting any treatments. Early identification of structural or traumatic causes of shoulder pain is essential to ensure safe and effective treatment.

Shoulder Pain Related
Many shoulder conditions, including frozen shoulder and rotator cuff strain, tend to feel worse at night, particularly when lying on the affected side. This is often due to increased pressure on inflamed tissue and reduced movement during sleep. If night pain is persistent or disrupting your sleep, it's worth getting it assessed.
Recovery time depends on the underlying cause. Muscle strain or tendinitis may improve within a few weeks with proper rest and physiotherapy, while frozen shoulder can take several months to fully resolve. A proper diagnosis helps set realistic expectations for your specific case.
In many cases, yes, but with guidance. Complete rest isn't always the best approach, as it can lead to stiffness. Our physiotherapists can advise on which movements are safe, which to avoid, and how to stay active without aggravating your shoulder.
Acupuncture may help some patients manage shoulder pain, muscle tension, and stiffness, particularly when used alongside physiotherapy and medical treatment. Suitability varies by individual, which is why our doctors assess each patient before recommending acupuncture.
Not always. Many cases of shoulder pain, such as muscle strain or mild tendinitis, can be assessed clinically without imaging. Our doctors will recommend an X-ray, ultrasound, or MRI only if needed, for example, if symptoms are severe, persistent, or suggest a more serious underlying issue.
Yes. Prolonged poor posture, especially from desk work or phone use, can contribute to muscle tightness and shoulder tendinitis over time. Our physiotherapists can guide you on posture correction as part of your treatment plan.
