Medical Concierge and Multigenerational Health Screening

Why Multigenerational Care Is Different

Health risks travel through families, genetically, environmentally, and through shared patterns of living. A cholesterol finding in one member is a data point. The same finding across three members is a signal worth acting on.

When a family's medical information is held together by one clinical team, hereditary risk patterns are mapped once and kept current, screening intervals are calibrated for each member, informed by the whole family's picture, and gaps in care become visible instead of hidden across multiple clinics and providers.

A grandfather's cardiac history shapes how we screen his adult children. A mother's thyroid condition informs what we look for in her daughters. This is what family medicine was meant to be. We built our practice for exactly this.

What We Provide

Bespoke screening for each family member

Every member receives a programme designed for their age, medical history, and risk profile, scheduled around the family's calendar, together where convenient and individually where not.

Screening is conducted by Dr Paul Chang, accompanied by trained clinic assistants. At your residence, we perform consultations, physical examinations, blood draws, and point-of-care testing. Imaging studies, X-ray, ultrasound, CT are arranged at our partner diagnostic facilities, coordinated entirely by our team.

Preventive care and ongoing medical management

Screening shows where each member stands. Preventive planning changes where they are heading.

Nutrition, physical activity, sleep, metabolic health, and cardiovascular risk are assessed and coordinated across family members, so that changes a family makes together actually hold. For members with existing conditions such as diabetes, hypertension, high cholesterol, or thyroid disorders, Dr Chang provides ongoing medication management, monitoring, and treatment adjustments as part of the engagement.

Integrative allied health

Where clinically appropriate, our in-house acupuncturist and physiotherapist work alongside Dr Chang as part of a coordinated care plan for pain management, musculoskeletal rehabilitation, post-surgical recovery, or preventive wellness. These are not separate referrals. They are part of the same team, sharing the same clinical picture.

Trusted specialist networks, with in-person accompaniment

When a family member needs specialist review, we do not simply issue a referral letter. Dr Chang can accompany the visit in person upon request (special billing rate applies), ensuring the history is communicated fully, the right questions are asked in the room, and the specialist's recommendations return to us for follow-through and implementation.

For elderly parents and family members who travel frequently, this is often the difference between an appointment and an outcome.

Discretion as Standard

Privacy is not a preference. It is a requirement.

Families are seen on private scheduling, outside of standard clinic flow where preferred. Dr Paul Chang remains the designated physician across all members, there is no retelling of histories to unfamiliar doctors.

Coordination with your family office is structured at the level of involvement you define. Some families prefer that we liaise directly with the family office executive. Others prefer all communication to remain between the physician and the principal. We follow your lead.

No information leaves the care team without explicit direction.

How an Engagement Begins

  1. Confidential consultation - A conversation with you or the principal to understand the family, members, existing conditions, concerns, and how care is preferred to be organised.
  2. Family health mapping - We chart the family's collective medical picture: hereditary risks, previous screenings, current gaps, and existing specialist relationships. This becomes the foundation for every clinical decision that follows.
  3. Individual programmes - Each member receives their bespoke screening and prevention plan, sequenced around the family's commitments. For families with members across different age groups, from young adults to elderly parents,  programmes are designed so that the clinical picture of one generation actively informs the care of the next.
  4. Ongoing stewardship - Results are reviewed, plans adjusted as lives change, and Dr Chang remains the single point of continuity, year after year. Annual reviews ensure screening stays current and care evolves with the family.
  5. Transparent fee structure - All services are billed based on the scope of care provided. House visits, specialist accompaniment, allied health sessions, and after-hours consultations are each itemised clearly. There are no bundled retainers with hidden inclusions or exclusions. You see exactly what each component of your family's care costs, and nothing is charged without prior discussion.

For Family Offices

If you are structuring health provision for a principal and their family, we work directly with family office executives as your clinical counterpart, providing the documentation, care planning, and coordination your role requires, while the family experiences nothing more clinical than a physician who clearly knows them.

We understand the operational cadence of a family office. Reporting is structured, scheduling is flexible, and communication follows whatever channel and frequency you establish. Whether you need quarterly health summaries for the principal, vaccination records for travel, or coordination across time zones for members based overseas, we handle it.

How does an engagement typically begin?

It starts with a private conversation, either with the principal directly or with the family office executive managing health provision. Dr Chang will want to understand the family: how many members, existing conditions, current medical relationships, and how care is preferred to be organised. From there, we map the family's collective health picture and design individual programmes for each member.

Can screening and consultations be done at our residence?

Yes. Dr Chang conducts consultations, physical examinations, blood draws, and point-of-care testing at your home. He is accompanied by trained clinic assistants. Imaging studies such as X-ray, ultrasound, and CT scans are arranged at our partner diagnostic facilities and coordinated by our team.

What if a family member needs to see a specialist?

We arrange the referral through our established specialist network and Dr Chang can accompany the family member to the appointment upon request. He ensures the medical history is communicated in full, participates in the consultation, and brings the specialist's recommendations back into the family's overall care plan. This is especially valuable for elderly parents or members who travel frequently.

Is the service limited to health screening, or does it include ongoing medical care?
It includes both. Beyond screening, Dr Chang provides ongoing management for chronic conditions such as diabetes, hypertension, high cholesterol, and thyroid disorders, including medication management and regular monitoring.
Do you work with family offices directly?
Yes. We work with family office executives as a clinical counterpart, providing health documentation, care planning, scheduling, and reporting at whatever level of involvement the family office defines. Some families prefer all coordination to go through their family office. Others prefer direct communication between the physician and the principal. We follow your lead.
How is privacy handled?
Families are seen on private scheduling, outside standard clinic flow where preferred. Dr Paul Chang remains the designated physician across all members, there is no rotation. No information is shared with any third party without explicit direction from the family. Coordination with your family office, insurers, or other parties happens only on your terms.
Are acupuncture and physiotherapy included?
These are available as add-on services where clinically appropriate. Our in-house acupuncturist and physiotherapist work alongside Dr Chang, sharing the same clinical picture. This is particularly relevant for elderly family members with chronic pain, musculoskeletal conditions, or post-surgical rehabilitation needs.
How many family members can be included?
We recommend the minimum number of family members to be 4.
