What Is the HIV 4th Generation Rapid Test?
The 4th generation HIV test, also called the HIV Ag/Ab combo test. detects two things simultaneously: the p24 antigen produced by the HIV virus itself, and the antibodies your immune system produces in response to HIV infection.
Because it detects the antigen (which appears earlier than antibodies), the 4th generation test has a shorter window period than older antibody-only tests. It can reliably detect HIV from 14-28 days after exposure (clinically conclusive at 45 days), compared to 90 days for the 3rd generation antibody test.
The rapid version of the 4th generation test uses a small fingerprick blood sample and returns results in approximately 20 minutes while you wait at the clinic. No blood draw from a vein is required, unless you are doing other tests.
When Should You Get Tested?
You do not need symptoms to get tested. In fact, many people living with HIV have no symptoms for years, testing is the only reliable way to know your status.
Consider getting tested if you:
- Had unprotected sexual contact with a partner of unknown HIV status
- Had a condom broke or was not used
- Has a current or previous partner has been diagnosed with HIV or another STI
- Had sexual contact with multiple partners
- Are starting a new relationship and want to confirm your status together
- Simply want peace of mind as part of routine health maintenance.
Regular HIV testing is also recommended for individuals who are on or considering PrEP (Pre-Exposure Prophylaxis). Quarterly testing is a required part of PrEP monitoring programme.
Understanding the Window Period
The window period is the time between potential HIV exposure and when a test can reliably detect the infection. Testing during the window period may produce a false negative — meaning the result comes back negative even though infection has occurred.
- 4th Generation Rapid Test (Ag/Ab): Reliable from 14-45 days after exposure. This is the test we offer at clinic with results in 20 minutes.
- 3rd Generation Antibody Test: Reliable from 90 days after exposure. This older test detects antibodies only and has a longer window period.
- HIV RNA PCR Test: Can detect HIV from as early as 10 to 14 days after exposure. This is a lab-based test (not rapid) with results in 2 to 5 working days. It is used when very early detection is needed.
If your last potential exposure was less than 14 days ago, the rapid test may not yet detect the infection. Our doctor will advise whether to test now and retest after the window period, or whether HIV PEP (Post-Exposure Prophylaxis) should be considered if the exposure was within the last 72 hours.

What Happens During the Test
The entire visit takes approximately 15 to 20 minutes.
- When you arrive - you will be seen by our doctor in a private consultation room. Our doctor will take a brief, confidential history, you will be asked about the reason for testing, approximate timing of last potential exposure, and any symptoms. This helps determine whether the rapid test is appropriate or whether a different test type would be more reliable based on your timeline.
- The test itself - involves a small fingerprick to collect a drop of blood. There is no needle in the arm and no blood draw from a vein. The sample is applied to a test cassette and you wait in the consultation room while it processes.
- Results are ready in approximately 20 minutes - Our doctor will interpret the result and explain what it means before you leave.
Confidentiality
While we are not an anonymous test clinic, all HIV testing at Regis Medical is confidential. Your results, consultation notes, and medical records are not shared with employers, insurers, or family members without your explicit written consent.
Under the Infectious Diseases Act, a confirmed HIV-positive diagnosis (after confirmatory testing) must be reported to the Ministry of Health by the testing laboratory. This notification is for public health surveillance purposes and uses a coded identifier. It applies to confirmed positive results only, not to negative results, and not to the act of getting tested.
Understanding Your Results
- If your result is negative - A negative result means HIV was not detected at the time of testing. If your last potential exposure was more than 14 days ago, this result is highly reliable. If your exposure was more recent, our doctor may recommend a follow-up test once the full window period has passed to confirm. A negative result does not protect you from future exposure. If you are at ongoing risk, our doctor may discuss regular testing schedules or whether PrEP (Pre-Exposure Prophylaxis) could be appropriate for you.
- If your result is positive - A positive result on the HIV rapid test is a preliminary finding, it is not a confirmed diagnosis. Our Doctors will recommend a follow up confirmatory lab test to rule out a false positive. Our doctor will explain this clearly. If the confirmatory test also returns positive, they will sit with you privately to discuss what this means, answer your questions, and walk you through next steps. These include referral to an HIV specialist for initiation of antiretroviral therapy (ART). Modern HIV treatment is highly effective. Most people on treatment achieve an undetectable viral load within months, meaning the virus is suppressed to a level where it cannot be transmitted to sexual partners. This is known as U=U (Undetectable equals Untransmittable) and is backed by extensive clinical evidence. A positive result is not the end of a conversation. It is the beginning of one.
- If your result is indeterminate - In rare cases, a rapid test may produce an unclear result. If this happens, our doctor will order a lab-based confirmatory test and explain what to expect during the waiting period.
Already on PrEP? You Still Need Regular Testing
If you are taking PrEP (Pre-Exposure Prophylaxis), whether prescribed in Singapore, purchased online, or obtained overseas, quarterly HIV testing is a mandatory part of safe PrEP use.
PrEP is highly effective at preventing HIV, but it must only be taken by people who are confirmed HIV-negative. If you become HIV-positive while on PrEP and continue taking it without knowing, the virus can develop resistance to the drugs, the same drugs used in first-line HIV treatment. This can seriously limit your future treatment options.
