Singapore is one of the most sleep-deprived nations in the world. Only one in four adults gets more than seven hours of sleep a night. The majority average around 6.4 hours, well below the recommended seven to nine. Forty per cent of Singaporeans wake up feeling tired every day, and stress is cited as the primary reason.
Most people understand that poor sleep makes them tired and less productive. Fewer understand that it is also raising their blood sugar. And even fewer realise that the combination of chronic stress and insufficient sleep creates a metabolic environment that actively drives insulin resistance, the precursor to prediabetes and Type 2 diabetes.
This is not a minor effect. Research shows that just one week of sleeping five hours a night significantly reduces insulin sensitivity in healthy adults. The metabolic impact of chronic sleep deprivation and sustained stress is comparable to some of the dietary and exercise factors that receive far more attention in diabetes prevention conversations.
Sleep is not passive rest. It is an active metabolic process during which your body regulates hormones, repairs tissue, and recalibrates its glucose management systems. When sleep is cut short, these processes are disrupted.
The most direct effect is on insulin sensitivity. During adequate sleep, your body maintains a healthy balance between insulin production and cellular response. When sleep is restricted, cells become less responsive to insulin. Glucose lingers in the bloodstream longer. Your pancreas compensates by producing more insulin, placing additional strain on beta cells that are already working to keep up.
Studies have demonstrated that even a single night of partial sleep deprivation raises fasting glucose the following morning. Extend that to weeks and months of sleeping six hours or less, and the cumulative effect is a measurable shift in your metabolic profile toward insulin resistance.
A systematic review of ten studies found that short sleep duration was significantly associated with insulin resistance and increased risk of metabolic dysfunction. People with prediabetes were more likely to report poor sleep quality than those with normal blood sugar, and they also showed higher levels of inflammatory markers, suggesting that chronic low-grade inflammation is one mechanism linking poor sleep to glucose dysfunction.
In practical terms: if you are sleeping six hours a night, exercising regularly, eating well, and still seeing your blood sugar creep upward, sleep may be the missing variable.
Cortisol is your body's primary stress hormone. It is released by the adrenal glands in response to perceived threat, whether that threat is a physical danger or a Monday morning inbox with 200 unread emails.
Cortisol's job is to make energy available quickly. It does this by signalling the liver to release stored glucose into the bloodstream and by reducing insulin sensitivity so that glucose remains available in the blood rather than being absorbed into cells. In an acute stress situation, this is useful. Your muscles need fuel. Your brain needs glucose. The system works as designed.
The problem is chronic stress. When cortisol remains elevated for hours, days, and weeks, the metabolic consequences accumulate. Blood sugar stays higher than it should. Insulin sensitivity remains impaired. The liver continues releasing glucose even when it is not needed. And cortisol promotes the storage of visceral fat around the abdomen, which itself worsens insulin resistance, creating a reinforcing cycle.
For Singaporeans, chronic stress is not an occasional inconvenience. It is a structural feature of daily life. Long working hours, high cost of living, academic pressure, caregiving responsibilities, and the cultural norm of being constantly available all contribute to sustained cortisol elevation. When this is layered on top of chronic sleep deprivation, the metabolic impact is compounded.
Sleep deprivation and chronic stress do not simply add together. They amplify each other.
Poor sleep raises cortisol levels - Elevated cortisol makes it harder to fall asleep and reduces sleep quality. Reduced sleep quality further raises cortisol. The cycle feeds itself, and each pass through the loop worsens insulin sensitivity a little more.
Research shows that sleep-deprived individuals also make poorer food choices. They gravitate toward high-carbohydrate, high-sugar foods because the brain, deprived of rest, craves quick energy. The hormones that regulate appetite (ghrelin and leptin) are disrupted by sleep loss, increasing hunger and reducing satiety. So not only is your body less able to process glucose efficiently, but you are also consuming more of it.

The American Academy of Sleep Medicine and the Sleep Research Society recommend a minimum of seven hours of sleep per night for adults. This is not an aspirational target. It is the threshold below which measurable metabolic harm begins.
In Singapore, only 44 percent of adults meet this recommendation. The remaining 56 percent are functionally sleep-deprived on a regular basis. This means more than half the adult population is living with some degree of metabolic impairment from sleep loss alone, before factoring in diet, exercise, genetics, or stress.
Seven to eight hours is the target. Quality matters too. Fragmented sleep (waking multiple times during the night) impairs glucose metabolism even if total hours are adequate. Deep, consolidated sleep is what your body needs to complete its metabolic housekeeping.
Addressing sleep and stress is not about dramatic lifestyle overhauls. It is about consistent, small adjustments that compound over time.
