Everyone knows that excessive alcohol damages the liver. But "excessive" is a word that means different things to different people. Two beers after work does not feel excessive. A bottle of wine shared over dinner does not feel excessive. Weekend drinks with friends do not feel excessive.
The question is not whether they feel excessive. The question is whether your liver agrees.
The liver processes approximately 90 per cent of the alcohol you consume. It does so dutifully, prioritising alcohol metabolism above virtually everything else it does. But this prioritisation comes at a cost, and that cost begins accumulating earlier than most people realise. Research shows that 90 per cent of people who regularly drink above recommended limits develop fatty liver, the first stage of alcohol-related liver disease. And fatty liver can develop after as little as two weeks of excessive drinking.
Two weeks. Not two decades.

If you regularly go above this limit, you are more likely to do yourself harm."
A standard drink in Singapore is defined as one small glass of wine (125 ml), half a pint of beer (250 ml), or one shot of spirits (30 ml). Each contains approximately 10 grams of pure alcohol.

A cocktail with double spirits is two to three. Three pints on a Friday night is six standard drinks in one session.
When assessed honestly against these definitions, many Singaporeans who consider themselves moderate drinkers are consuming 15 to 25 units per week. This places them in the hazardous range where liver damage risk rises meaningfully.

The liver's response to alcohol follows a predictable progression, but the timeline is faster and more individual than most people expect.
Within days to weeks. When you drink more than your liver can process efficiently, it begins storing the excess metabolic byproducts as fat. This is the earliest stage of alcohol-related liver change. It can begin after just two weeks of regularly exceeding the recommended limits. At this stage, there are no symptoms. Your liver enzymes may or may not be elevated. The fat is simply accumulating silently.
Within months. With continued drinking above safe limits, fatty liver becomes established. Approximately 90 per cent of heavy drinkers develop this stage. It is still entirely reversible if drinking is reduced or stopped. Most people at this stage feel completely normal and have no idea their liver has changed.
Within years. If heavy drinking continues, the accumulated fat triggers an inflammatory response. Liver cells become inflamed and damaged. This is alcoholic hepatitis (not to be confused with viral hepatitis). Symptoms may include fatigue, abdominal discomfort, nausea, and loss of appetite, though many cases remain asymptomatic. The threshold for high risk of alcoholic hepatitis is generally considered three to four drinks per day over an extended period. Alcoholic hepatitis is still reversible with sustained abstinence, but the window is narrowing.
Within a decade or more. Chronic inflammation produces scar tissue (fibrosis), which eventually progresses to cirrhosis: permanent, irreversible scarring that distorts liver architecture and impairs function. Approximately 30 per cent of heavy drinkers progress to this stage. Those who develop cirrhosis have typically consumed more than six standard drinks per day for 10 or more years. Cirrhosis cannot be reversed. It can be managed, but the damage is permanent.
This progression is not inevitable. Not every drinker progresses through every stage. But it is dose-dependent and duration-dependent: the more you drink and the longer you drink, the further along the spectrum you travel.
The same amount of alcohol damages some livers more than others. Several factors determine individual vulnerability.
Sex. Women develop alcohol-related liver disease at lower levels of consumption than men. Women produce less of the enzyme alcohol dehydrogenase, which breaks down alcohol in the stomach before it reaches the liver. They also have lower body water content, meaning the same number of drinks produces higher blood alcohol concentrations. The risk thresholds for women are roughly half those for men.
Pre-existing metabolic fatty liver. This is the compounding factor that is most clinically relevant in Singapore. If you already have metabolic fatty liver (MASLD) from insulin resistance, visceral fat, and a high-carbohydrate diet, adding alcohol creates a double burden. Your liver is already struggling with metabolic fat. Alcohol adds direct toxic damage on top. A Harvard study found that overweight women who drank heavily had three times the risk of cirrhosis and liver-related death compared to light drinkers of normal weight. The combination of metabolic dysfunction and alcohol is far more dangerous than either alone.
Given that 40 per cent of Singaporeans in a community study had fatty liver, this compounding effect is relevant to a substantial proportion of the population who drink moderately without realising their liver is already under metabolic strain.
Genetics. Variations in alcohol-metabolising enzymes (common in East Asian populations, including the ALDH2 deficiency that causes the "Asian flush") affect how efficiently alcohol is processed. Some genetic variants increase the accumulation of acetaldehyde, the toxic intermediate metabolite, prolonging liver exposure to its damaging effects.
Hepatitis B. Singapore has a significant hepatitis B carrier rate. Chronic hepatitis B combined with regular alcohol consumption dramatically accelerates liver damage compared to either factor alone. If you are a hepatitis B carrier, there is no safe level of regular alcohol consumption.
Obesity. Excess body weight independently increases the liver's vulnerability to alcohol-related damage through shared inflammatory pathways and increased hepatic fat burden.
The early stages of alcohol-related liver disease are invisible without testing. As we explained in our blog on liver markers, specific patterns in your liver function test can indicate alcohol-related changes.
Elevated GGT is the most sensitive marker of alcohol's effect on the liver. GGT rises in response to regular alcohol consumption, often before ALT or AST become abnormal. If your GGT is elevated and you drink regularly, the association is likely causal.
AST higher than ALT (AST/ALT ratio greater than 1). This pattern is characteristic of alcohol-related liver injury, particularly when the ratio exceeds 2. It reflects the preferential mitochondrial damage that alcohol produces.
Elevated ALT. Indicates active liver cell damage. When elevated alongside a history of regular drinking, it confirms that the liver is being actively injured.
Macrocytosis (raised MCV on full blood count). Alcohol impairs vitamin B12 and folate absorption, producing larger-than-normal red blood cells. This is often the incidental finding that prompts a doctor to inquire about alcohol intake.
If your last health screening showed elevated GGT or an AST/ALT pattern consistent with alcohol effect, and you drink regularly, the test result is not an isolated data point. It is your liver telling you where it stands on the spectrum.
The good news is that liver recovery can begin remarkably quickly.
Liver function tests begin improving within two to three weeks of reducing or stopping alcohol. Fatty liver can resolve completely within weeks to a couple of months of abstinence. Inflammation (alcoholic hepatitis) typically improves within weeks to months of stopping. Early fibrosis can improve over months to years with sustained abstinence, though recovery is slower and less complete than at earlier stages.
This recovery capacity is one of the liver's most remarkable features. But it is not unlimited. Each cycle of damage and recovery leaves the liver slightly more vulnerable. And beyond a certain threshold of scarring, the damage becomes permanent.
If you drink regularly and have not had your liver function tested, you are making assumptions about an organ that produces no symptoms when it is under strain.
